The situation in the UK is only slightly better, with one in five patients experiencing some error. Sadly, it was also found that Britons are the least likely to report an error, so the true picture may well be worse.

These were the results of a survey by the Commonwealth Fund that involved 2200 people who had recently been in hospital or who had health problems (Taking The Pulse of Health Care Systems; http://content.healthaffairs.org).
