A phrase that includes something about stable doors and bolting horses comes to mind as we read that the UK is launching the world's first system to capture medical errors.

This trail-blazing organization will be known as the National Reporting and Learning System (NRLS). It will collect reports from 'health professionals' across England and Wales.

In launching the initiative, Susan Williams, chief executive of the new agency, says that medicine is drawing from the safety recording of the aviation industry. This seems unlikely as the number of fatalities from an adverse reaction to a drug equates to a jumbo jet crashing every day. Such a supposed 'safety record' would mean that nobody would fly again.
